Licensing Package

The Licensing Package is designed to assist the applicant in transitioning a residential home to a licensed AFH and prepare the client to operate the AFH. This package has two phases and is designed to take you through the licensing process in manageable steps.

Phase One

Phase one is $575 includes the following:

  • Initial consultation via Zoom video call prior to inspection to:
    • Explain the process of becoming a provider or converting your home to an AFH.
    • Identify where you are as far as meeting the AFH provider qualifications.
    • Address any questions you may have about the process.
  • A thorough virtual inspection of the home and property identifying areas that will need changes to meet AFH licensing requirements. This includes measuring rooms to determine if they are single capacity or double.
  • Contacting your local building official to determine the process and potential costs to complete the AFH Local Building Inspection – often referred to as the “WABO”. This process is different in every jurisdiction in the state.
  • Meeting with your architectural designer and/or contractor who is remodeling the home to ensure modifications are completed according to building code specifications.

Phase Two

Phase Two is $1,150 and includes the following:

  • Completing and submitting the AFH application
  • A series of Zoom meetings to help you develop the required documents for your home which includes:
    • Admission Agreement
    • Disclosure forms (Disclosure of Service and Charges) and Medicaid Policy
    • Policy Manual – includes required policies such as your disaster plan, Abuse and Neglect, Infection Control, L&I Accident Prevention, and Respiratory Protection Program
    • How to admit residents and Hire staff
    • Forms for the operation of your home
  • Once you have completed your Zoom series, we develop your filing system to help you organize your records and setup your binders.
  • A virtual walk-through visit(s) at the home – ensuring areas meet licensing requirements and everything is ready for the initial licensing inspection with DSHS. This is a less thorough walk-through than Phase One and does not include measuring bedrooms.
  • Once you have your licensing date from DSHS, we set up a final Zoom call to go over everything you have learned and get you ready for licensing day!
  • After your license is approved, we will provide a follow-up Zoom meeting to:
    • Go over your documents and checklists to prepare you for admitting your first resident; and
    • Reconcile resident’s medications.

Optional Services for Phase Two:

  • Ordering supplies: We will order your required supplies and have them delivered to you. This includes things like: fire extinguisher/s, binders for your records, call bell systems, etc. You will be billed separately for the supplies and there is a $50 convenience fee if we order them for you.
  • Register the LLC, EIN and business license through Department of Revenue $200

Change of Ownership (CHOW) Licensing Package

The Change of Ownership (CHOW) Licensing Package is designed for those clients who are licensing a currently licensed AFH. This package is designed to take you through the licensing process in manageable steps and transition the home from the prior provider to you.

CHOW Package

Phase One is $975 and includes:

  • Completing and submitting the AFH application
  • A series of Zoom meetings to help you develop the required documents for your home which includes:
    • Admission Agreement
    • Disclosure forms (Disclosure of Service and Charges) and Medicaid Policy
    • Policy Manual – includes required policies such as your disaster plan, Abuse and Neglect, Infection Control, L&I Accident Prevention and Respiratory Protection Program
    • How to admit residents and Hire staff
    • Forms for the operation of your home
  • The tab dividers for the binders, which are setup to work with your Policy Manual.
  • Zoom meeting(s) to discuss changing your records over from prior provider, such as:
    • Resident records
    • Staff records; and
    • Nurse delegation, etc.
    • What records should you keep and what not to keep
    • Reconcile resident’s medications
